Getting Started
We have made a comprehensive list of our enrollment forms, links to state childcare assistance forms, and safety and health forms for your child. ie, Allergies, medications and emergency contact forms or anything that we (the state) feels we need to know about your child.
Below are State REQUIRED documents. If these forms are not finished and submitted you will be denied childcare. Some of the forms are state forms. They must be finished via the PDF's below and uploaded or printed and filled out by hand.

Request for Immunization Records (you can also request this from you physician or the county health nurse)
Unfortunately the above forms are not mobile friendly, if you do not want to print them out you must complete them on a computer. If you need help uploading the forms when completed please contact us, we are here to help!
Some of this information may be redundant. The following forms help us keep all information current, and determine if the parents have turned in all necessary information for the state. The forms above MUST be uploaded to our new Child registration form. If you are having issues please contact us and we will assist you in this process.
If you are looking for temporary or emergency childcare please fill out the above forms and upload them to a completed Day Pass form.
We understand that there is a lot of paperwork required and we are here to help in any way possible. We have done our best to streamline the process for our parents, but If you have any questions about any of the paperwork please dont hesitate to reach out.

There are some additional things that must be filled out in person and we will send you a description of thoes when we send our policy and contract. If you would like to see the contract or policy prior to joining Little Rascals feel free to stop by and speak to us!
The following form is not required unless requested by the state. You may submit it with the rest of your paperwork if you would like.
